These are general expectations after an uncomplicated diagnostic angiogram. Your written discharge instructions take priority because the access site, closure method, medicines and your health can change recovery. Stenting or embolization requires a separate treatment-specific plan.
What is a cerebral angiogram?
Cerebral catheter angiography, also called digital subtraction angiography or DSA, uses a thin catheter, X-rays and iodine-based contrast to show the arteries and blood flow around the brain. The catheter usually enters through the wrist or groin; the procedure does not require opening the skull.
CTA and MRA are less invasive scans that often answer the initial question. DSA may add the fine vessel detail or flow information needed to clarify a finding or plan treatment. Your physician weighs that added information against the procedure's risks.

The benefit is additional information that may change your care. Catheter angiography also has risks: bleeding or a collection of blood at the puncture site, injury or blockage of a blood vessel, contrast reaction, kidney injury and an uncommon but serious risk of stroke that can cause lasting disability. The test uses X-rays; sedation has its own risks, including breathing problems.
Before you consent, your physician will explain your individual risk, the question the test is intended to answer, and whether CTA, MRA, follow-up imaging or another approach could answer it. Ask how the result would change your treatment plan.
Before your procedure
- Confirm your arrival time, procedure location and exact eating and drinking instructions. Fasting depends on the planned sedation or anesthesia; do not assume another patient's instructions apply to you.
- Bring a current list of prescription medicines, over-the-counter medicines and supplements. Ask for a written plan for any medicine to hold and the exact restart time.
- Tell the team about prior contrast reactions, kidney disease, bleeding problems, pregnancy possibility, diabetes, sleep apnea and previous anesthesia problems. Complete any requested blood tests.
- Arrange a responsible adult to take you home and stay with you for the first night; many units request 24 hours of support. An unaccompanied rideshare is not a substitute for the required escort.
- Have your prior imaging available, along with your questions, medication list and the phone number for your ride. Plan a quiet recovery day.
What happens on the day?
Check-in & questions
The team reviews your history, medicines and consent. An IV and monitoring equipment may be placed.
Numbing & imaging
Local anesthetic numbs the access site. Sedation may help you relax; some patients need general anesthesia. The physician guides a catheter to the vessels supplying the brain, injects contrast and records images. Brief warmth is common.
Protecting the access site
After catheter removal, pressure, a wrist band or an arterial closure device helps stop bleeding. Staff monitor your neurological status and the puncture site.
Going home
At our ambulatory surgical facility, recovery-room monitoring usually lasts 1–2 hours after a diagnostic cerebral angiogram. Once the care team confirms that you meet discharge criteria, you go home the same day with your escort. We do not provide overnight stays. Your total visit includes check-in, preparation, the procedure, and recovery; allow more time than the recovery period alone.
Wrist or groin: why the access site matters
Wrist / radial access
A compression band usually protects the wrist while the artery seals. You may be able to walk sooner, but protect that arm from lifting, forceful gripping, pushing or pulling until the team clears it. Follow the specific wrist-care instructions you receive.

Groin / femoral access
You may need to lie flat and keep the access-side leg straight for the period your team prescribes. The required time depends on how the artery was closed. Once cleared to get up, begin with gentle movement and avoid straining the groin.
Recovery at a glance
Monitoring usually takes 1–2 hours. The team checks your neurological status, access site, vital signs, and readiness to go home. Discharge depends on your assessment, not only the clock. If you need care beyond the facility’s capabilities, the team arranges the appropriate level of care.
There is no overnight stay at the facility. Arrange an adult escort and support at home for the first night, or longer if your discharge instructions require it.
Recovery begins in the procedure unit. Wait for the team to clear you before standing or walking. The notes below are typical expectations for an uncomplicated diagnostic study; your own instructions set the limits.
Confirm the plan.
Know how to protect the wrist or groin, which medicines to take, and who to contact overnight. Leave with your escort.
Keep the day quiet.
Rest with support at home. Once cleared, take short, gentle walks. Follow sedation restrictions and do not drive yourself home.
Build back gradually.
Keep heavy lifting and strenuous exercise on hold for the period on your discharge sheet—often at least 48 hours. Confirm your shower, work, and travel plan.
Do not independently stop or restart aspirin, clopidogrel (Plavix), anticoagulants, or diabetes medicines. Confirm the exact instructions with the procedure team before fasting and again before discharge.
Know when to get help
Call 911
Call 911 for new facial droop, one-sided weakness or numbness, trouble speaking, sudden vision loss, a sudden severe headache, fainting, chest pain or trouble breathing. Do not drive yourself or wait for an office callback—even if neurological symptoms improve.
For heavy or persistent puncture-site bleeding, lie down and apply firm, continuous pressure at the site while someone calls 911. Keep pressure on while waiting for help. Rapidly growing swelling, or a newly cold, pale, painful or numb hand or foot also needs emergency assessment.
Call your procedure team promptly
Contact the procedure team promptly for increasing redness, warmth, drainage, fever, worsening site discomfort, a new rash without breathing symptoms, or uncertainty about your medicine plan. Use the after-hours number on your discharge paperwork when the office is closed.

Common questions after an angiogram
Can I walk after my angiogram?
Yes—after the recovery team clears you to get up. Start with short, easy walks around home. Rest between them. Avoid long walks, brisk exercise and straining for the first 48 hours, or longer if your discharge plan says so.
Can I use stairs?
If you are steady and have been cleared to walk, use necessary stairs slowly. Limit repeated trips for the first 24–48 hours, especially after groin access. After sedation, avoid stairs or have someone help if you feel unsteady. Have someone carry bags or laundry.
What about standing, sitting, bending or squatting?
After prescribed bed rest ends, change position gently and take breaks. Avoid repeated deep bending, squatting or movements that strain the puncture site early in recovery. Stop if a movement causes access-site pain, swelling or bleeding.

When can I lift, exercise or return to the gym?
Heavy lifting and strenuous exercise are commonly restricted for at least 48 hours and sometimes several days. Ask for your exact lifting limit and return date, especially for weight training, running, cycling or a physical job. Wrist access also requires protecting that arm.
When can I go back to work?
Some people return to light or desk work after 24–48 hours if they feel well. Jobs involving lifting, driving, prolonged physical effort or safety-sensitive tasks may need more time. Request a work note with restrictions before discharge.
When can I drive?
Do not drive yourself home. Published instructions commonly restrict driving for 24–48 hours after diagnostic angiography; sedation, symptoms or treatment can extend this. Resume only when your discharge plan permits and you can operate the vehicle safely.
Can I take a shower?
You may shower gently once your procedure team permits it. Use lukewarm water, avoid scrubbing or directing a strong spray at the puncture, and pat dry. Follow the exact timing and dressing instructions on your discharge sheet.

When can I take a bath, swim or use a hot tub?
Avoid soaking the puncture site until it has healed and your team permits it. A shower and immersion are different: being allowed to shower does not automatically mean a bath, pool or hot tub is safe.
How do I care for the dressing and puncture site?
Keep the area clean and dry. Follow your team's dressing-removal instructions and check the area daily. Do not pick at skin glue or a scab. Ask before applying creams, ointments or powders.

Is bruising or a small lump normal?
A small, stable bruise and mild tenderness can occur. Some closure methods leave a small lump. Increasing swelling, worsening pain, expanding bruising or a lump that grows needs prompt assessment; heavy bleeding is an emergency.
Can I travel by car or fly afterward?
Arrange a responsible adult to drive you home after discharge. Discuss long drives and flights before the procedure. Clearance depends on your diagnosis, sedation, access-site recovery, and whether treatment was performed. If you live far away, ask whether you should recover at a nearby home or hotel with your escort. Our ambulatory facility does not provide overnight stays.
When can I have sex?
Treat sexual activity like other physical exertion. Avoid it during the early activity restriction—commonly at least 48 hours after groin access—and wait longer if it strains the puncture site or your physician gives a longer restriction.
Should I stop aspirin, Plavix or a blood thinner?
Do not stop, skip or restart an antiplatelet or anticoagulant on your own. These drugs have different purposes. Your procedure team must give you a specific plan; after a stent, prescribed antiplatelet treatment can be especially important.
What about insulin, metformin or other diabetes medicines?
Ask for an individualized plan before fasting. Insulin and other diabetes medicines may need adjustment. Metformin instructions depend on kidney function and the catheter procedure; if it is held, confirm when to restart and whether a kidney blood test is needed.
How much should I drink, and what can I eat?
Once the team allows it, resume food as tolerated and drink fluids regularly. Do not force large amounts of water if you have heart failure, kidney disease or a fluid restriction. Follow the amount recommended for you.
What about pain medicine, alcohol or sleeping tablets?
Use only the pain medicine approved on your discharge plan; check before adding ibuprofen, naproxen or extra aspirin. For 24 hours after sedation, avoid alcohol and recreational drugs, and do not take sleeping tablets unless your clinician specifically approves them. Do not drive, operate machinery or make important decisions during that time.
Will I be awake, and how long does it take?
Many diagnostic angiograms use local anesthetic with light or moderate sedation. Your team will explain your anesthesia plan. At our ambulatory surgical facility, post-procedure recovery usually takes 1–2 hours. You go home the same day once cleared; there is no overnight facility stay. Allow additional time for preparation and the angiogram itself.
When will I receive results?
Your physician may discuss initial findings before you leave. A final review or discussion with other specialists may follow. Confirm how you will receive the report and when to review the findings, treatment options and any follow-up imaging.
Are these instructions the same after embolization or a stent?
No. A diagnostic angiogram gathers information. Embolization or stenting treats a condition and may require different monitoring, medicines, activity restrictions and follow-up. Follow the discharge instructions for the treatment you actually received.
What are the risks, and what if I notice new symptoms?
Risks include bleeding or vessel injury, contrast reaction, kidney injury and an uncommon but serious risk of stroke. Your individual risk depends on your health and the procedure. Use the urgent-help guidance above for new neurological symptoms or significant bleeding.
