Confirm the suite, parking, building entry, and accessibility needs. Consultation offices and procedure sites may differ. Please confirm insurance participation and any referral requirements when scheduling.

Office hours are Monday–Friday, 8 am–5 pm Pacific.

Visit checklist

Use this checklist to prepare. Your selections are not sent to the office.

• Photo identification and insurance information
• Referral or authorization, if your plan requires one
• Imaging reports and image files, hospital records, and relevant test results
• A current medication list, including blood thinners
• Questions you want to discuss
• Asked the office how to transfer medical records securely
• A responsible adult and transportation if sedation may be used
